RN Case Manager

St. Peter’s Health Partners Medical Associates seeks a full time RN Case Manager to join their growing team.

The Patient Centered Medical Home RN Case Manager assumes the responsibility and accountability for the care coordination of the patients in the primary care practices they are assigned to. The RN CM serves as a resource and expert to the staff and provides on going education to staff, patients, and families. Nurses in this position work within and contribute to an environment where the SPHPMA mission is actualized, patient focused outcomes are achieved, and high quality professional practice is realized.

Requirements:
– Current NYS RN license, BSN degree is preferred and/or certification as a Case Manager (CCM).
– Proficient computer skills
– Experience with electronic health records are required
– Two-to-five years’ experience in a clinical setting managing chronic diseases, case management, and utilization management.
– Creative problem solving skills
– Strong communication skills
– Experience in a team environment

Knowledge of Patient Centered Medical Home concepts are preferred.
